IMPORTANT: Please be advised that our processing time is typically two business days upon receipt of correct documentation.

The list below includes all client forms, including those used for establishing a new account, making changes to your account, or conducting a transaction. All forms contain fill-in fields, but must then be printed and signed. Once you have opened or downloaded the form, you may type into the fields you would like to complete, then print and sign each form. Some forms may be faxed or emailed to our office once they have been signed. Other forms require the original to be submitted. See the details of each form for more information.

Account Management

Form Description
Application Use this General Application to open a Traditional IRA, Roth IRA, SEP IRA, SIMPLE IRA, or Health Savings Account. Please complete all sections completely and download the appropriate disclosure document for the type of account you are opening. Use a separate application for each account you wish to open. Be sure to include a legible copy of your photo ID when establishing a new account. (Original Required)
Client Release Form REQUIRED FOR ALL NEW ACCOUNTS. This form confirms your understanding of certain important aspects of your self-directed account, and must be witnessed by a notary. (Original Required)
5305 SEP Use this additional required form only if you are opening a SEP (Simplified Employee Pension) IRA to establish the plan eligibility rules. (Original Required)
Coverdell ESA Application Use this Application to open a Coverdell Education Savings Account (ESA). Please complete all sections completely and download the 5305EA disclosure for your records. Be sure to include a legible copy of the Depositor’s photo ID when establishing a Coverdell. (Original Required)
Interested Party Designation/Limited Power of Attorney Use this optional form to give other individuals certain rights pertaining to your account. By completing the Interested Party section, you can allow our office to provide information about your account (such as balances, or the status of transactions) to the Interested Party. By completing the Limited Power of Attorney section you are authorizing that individual to conduct transactions within your account. In either case, the form must be witnessed by a notary. Beneficiary Designation/Change Use this form to designate or change the beneficiaries for your account. This form is not required when opening a new account, as the information is incorporated into the application. Use this form to name or change beneficiaries at any time after your account has been established. (Original Required)
Change of Address Form Use this form to update or change the address on file for your account. All acceptable “Proof of Address” types are noted on the form. (Fax/Scan Accepted)

Fee Information

Form Description
Fee Schedule REQUIRED FOR ALL NEW ACCOUNTS. Use this form to indicate your choice of annual fee structure, as well as your billing preferences for the account. Be sure to review this carefully, as it contains information about all fees that could apply to your account. (Original Required)
Credit Card Authorization Use this optional form if you want to use a credit card to pay your account fees. The form can be used for one-time or recurring payments. (Fax/Scan Accepted)

Funding Your Account

Form Description
Deposit Coupon  Use this form if you are making a deposit to your account, including annual contributions and income from assets held in your account (such as loan payments or rental income). Do not use this form for deposits related to rollovers, Roth Conversions, or the sale of an asset within your account. (Fax/Scan Accepted)
Transfer Form 2014-07-16 Use this form to move funds or other assets from an account at another institution to the same type of account at Next Generation. This type of fund movement is not reported to the IRS and can be done as often as you would like. Remember to check with the other institution to find out if they require a Medallion Guarantee in order to release your funds. We will require a current account statement from the relinquishing custodian in order to process. (Original Required)
Rollover Certification  Use this form to confirm the source of funds or other assets being rolled over into your Next Generation account. This may be used for funds coming from Employer Plans (such as 401k) as well as from other IRAs. This type of fund movement is reported to the IRS. Next Generation will not initiate this movement of funds, so you must contact the other institution directly. Funds Rolled over from an IRA must be put back into an IRA within 60 days, and this type of rollover can only be done once per 12 months from that account. (Fax/Scan Accepted)
Traditional to Roth Conversion Request Use this form to request a Conversion to a Roth IRA. If you are converting funds or other assets from an existing Next Generation account, you must have a Roth IRA established in order to complete the conversion. If you do not already have a Roth IRA at Next Generation you will need to complete the new account documents. This form is also used to document funds or other assets coming from another institution for deposit as a Conversion to your Roth IRA at Next Generation. (Fax/Scan Accepted)

Transaction Documentation

Form Description
Buy Direction Letter Use this form to instruct Next Generation to make any investment within your account. Complete the appropriate sections of the form depending on the type of asset that you are purchasing. Please note that additional supporting documents are usually required, but vary depending on the type of investment. Please speak with an Account Representative to confirm what information will be necessary to process your request. (Fax/Scan Accepted)
Private Placement Instruction Letter Use this form in conjunction with a Buy Direction Letter and other supporting documents if you are instructing Next Generation to invest your account into any sort of Private Placement or other private entity, such as an LLC, LP, Private Stock, Hedgefund, etc. (Fax/Scan Accepted)
Private Placement Investor Advisory Notice Use this form in conjunction with a Buy Direction Letter, Private Placement Instruction letter and other supporting documents if you are instructing Next Generation to invest your account into any sort of Private Placement or other private entity, such as an LLC, LP, Private Stock, Hedge Fund, etc. This form is to be completed, signed and dated by an eligible party of the investment entity.  (Fax/Scan Accepted)
Commodities Futures Instruction Letter Use this form in conjunction with a Buy Direction Letter and other supporting documents if you are instructing Next Generation to purchase Commodity Futures Contracts, including Foreign Exchange Contracts within your account. This includes the establishment of trading accounts for these assets. (Fax/Scan Accepted)
Payment Authorization Use this form to instruct Next Generation to pay expenses associated with any asset in your account. Remember that all asset-related expenses must be paid from your account, and cannot be paid personally out of pocket. You should attach any invoice or other supporting information to this form. (Fax/Scan Accepted)
Fair Market Valuation Form Use this form to report changes to the fair market value of assets held in your account. Each account holder is responsible for providing an annual report of the fair market value of all account assets to Next Generation. This form may also be needed when performing a Roth conversion of assets, or if taking a distribution of assets in-kind. This form must be submitted with supporting documents. (Fax/Scan Accepted)

Sell Direction Letter Use this form to instruct Next Generation to sell or liquidate any investment within your account. Complete the appropriate sections of the form depending on the type of asset that you are selling/liquidating. Please note that additional supporting documents may be required, but vary depending on the type of investment. Please speak with an Account Representative to confirm what information will be necessary to process your request. (Fax/Scan Accepted)

Outgoing Wire/ACH Instructions
Use this form to provide us with outgoing Wire or ACH (direct deposit) instructions for funds going from your account to anyone. This includes purchases, payments, as well as distributions to you directly. Be sure to get this information from the bank where funds are being sent, in order to prevent misdirected funds. (Fax/Scan Accepted)
Distribution Form Use this form to request distributions of cash or assets from your account. Note that any distribution will be reported to the IRS and may be a taxable event, depending on the type of account and reason for distribution. Feel free to contact our office for general information, or you may contact your tax advisor for more information about the specific tax effects for your situation. (Original Required)
